Our Home Health Care department can provide a full spectrum of health care services right in our client's homes.
Our highly-trained staff of nurses and other professionals care for you by visiting you at your home or current
place of residence. Our main purpose is to provide skilled nursing care and therapeutic services on an intermittent basis.
Clients are generally referred by their physicians. Admission to Home Health Care is based on the client’s need
for skilled nursing services and the agency’s ability to meet those needs safely. In collaboration with you and
your physician, we will coordinate a program of the various disciplines specific to meet your needs. In
addition to providing direct patient care, we offer health teaching to assist you to increase your knowledge
on your current health status and to improve your self-care ability to maintain your health and prevent further
illness. Your physician and you decide how we can best care for you. We provided your physician with regular
reports on your health status.
We provided Home Health Care service to residents in Jefferson County and parts of Clarion, Forest, Elk,
Armstrong and Clearfield counties. A service area listing is available upon request.

Physicians
- Dr. Gary Haverty, D.O. is Medical Advisor for our home health care department. He also serves as the
physician representative on our Advisory Group of Professional Practice Committee.
Any physician with a current and active PA state license that is free of disciplinary action may refer a
patient for home care services.
Staff
- As applicable, all clinicians are required to maintain a current and active PA state license by the
PA Department of Occupational and Professional Affairs.
Several of the nursing staff members hold additional levels of accreditation in the following areas: OASIS,
Coding, Psychiatric and Gerontological Nursing.

- Charitable fund established in 1986 after a donation was received by the Redbank Ministerial
Association. The monetary gifts received are used for the enhancement of patient care for home health
